Eden Brown Synergy are currently recruiting for an experienced Qualified Social Worker to join the Children with Disabilities Team within Achieving for Children on a permanent, full-time basis. Duties and Responsibilities: Assess the needs of children with disabilities and their families. Develop and implement care plans, chair meetings, and ensure all actions remain on track. Work closely with parents, carers, and partner agencies to promote positive outcomes. Utilise a range of interventions to support children and ensure their voice is central to care planning. Contribute to the development of service projects and support colleagues through mentoring or supervision. Liaise with local specialist schools and other community resources.
